Important Information for Class Booking

Before booking your Forklift Licence (LF) Training, please carefully review the following entry requirements, enrolment requirements and course information. Please make sure you meet the entry requirements and can complete all required pre-course activities before attending your class. 1. ENTRY REQUIREMENTS To enrol in this course, you must: • Be at least 18 years of age; • Have the right to study or work in Australia; and • Have a valid Photo ID. 2. COMPLETE ENROLMENT BEFORE YOUR CLASS After booking, you will receive email instructions to create your Online Learning Platform (OLP) account. Before attending your class, you must complete: • Enrolment Form; • ID Upload; and • Learning Competency Assessment. The Learning Competency Assessment includes course instructions and Language, Literacy and Numeracy (LLN) assessment. You must have sufficient English and LLN skills to understand safety instructions, read and write in English, communicate verbally, and complete simple calculations required during training and assessment. 3. COMPLETE ONLINE THEORY LEARNING AND ASSESSMENTS You must complete all required online theory learning and assessments before attending your class. These activities provide the essential knowledge you need before commencing face-to-face practical forklift training and assessment. 4. ATTENDANCE Please arrive on time for your scheduled class. Entry may be refused if you arrive 15 minutes or more after the scheduled starting time. Please check your Booking Summary and Booking Confirmation Email for the correct training address. Do not rely solely on Google Maps, as Origin Institute may operate multiple training locations within the same suburb. 5. PHOTO ID You must bring your original valid Photo ID to class. Your identity will be verified before training and assessment. An assessment outcome cannot be issued if your identity cannot be properly verified. 6. UNIQUE STUDENT IDENTIFIER (USI) All participants undertaking nationally recognised training must have a Unique Student Identifier (USI). If you do not have a USI, you can create one at www.usi.gov.au. 7.
